India has made significant strides in health research, yet a persistent gap remains between what research evidence recommends and what happens in clinical practice and public health programs. Studies estimate that it takes an average of 17 years for research evidence to reach routine clinical practice. In resource-constrained settings like India, this gap can be even wider.
What is Implementation Research?
Implementation research (IR) is the scientific study of methods to promote the uptake and integration of evidence-based practices into routine healthcare. Unlike traditional clinical research that asks "Does this intervention work?" (efficacy), implementation research asks "How can we make it work in the real world?" (effectiveness and implementation).
Key Implementation Frameworks
Several frameworks guide implementation research:
- RE-AIM: Evaluates Reach, Effectiveness, Adoption, Implementation, and Maintenance of interventions
- CFIR: The Consolidated Framework for Implementation Research identifies factors across five domains that influence implementation
- Proctor's Taxonomy: Distinguishes between implementation outcomes (acceptability, adoption, fidelity), service outcomes (efficiency, equity, timeliness), and client outcomes (satisfaction, function, health status)
Implementation Research in Indian Context
India offers a unique context for implementation research due to its diverse healthcare delivery system (public, private, traditional medicine), massive community health worker cadre (ASHA workers, ANMs, AWWs), varied geographic and cultural contexts, significant urban-rural health disparities, and ongoing national health programs (NHM, Ayushman Bharat, NPCDCS).
